Civilian Amputee Assistance helps amputees, or those born without a limb(s), with some of their costs when they need to go to an Artificial Limb Centre.

How to apply for Civilian Amputee Assistance

We can grant you the financial assistance you qualify for from the date you first contact us if you complete your application within 20 working days of that date.

You can call us and we'll talk with you to understand your situation and how we can help. We'll let you know what information you need to provide and how to get this to us.

You also need to fill out a form

We'll ask you to complete a Civilian Amputee Assistance form and we'll discuss the best way to get this to us.
